POSITION:Public Health Nurse (Nurse-Family Partnership Grant)
DEPARTMENT:Health
SALARY:$48,722 – 53,595, depending on relevant experience
RESPONSIBILITIES:
Provision of services to meet the objectives of the Nurse-Family Partnership Grant.Case manages and performs nurse home visits to first time, low income, and high-risk mothers.
EDUCATION/KNOWLEDGE/SKILLS:
Bachelor’s Degree in Nursing with a minimum of two years experience.
Community health field experience preferred
PA certification as a registered nurse
Bilingual ability preferred
Knowledgeable in all areas of maternal child health and nurse home visiting
Excellent written and oral communication skills
Excellent problem solving and decision making skills
Excellent knowledge of computers, including spreadsheets and data bases.
Ability to work independently and travel throughout the County
Ability to collaborate with other organizations throughout the County
Ability to commit to a four (4) year grant funded position
Ability to attend trainings and meetings in and out of State
Performs tasks or other responsibilities as assigned.
